The Mission
Named in honor of a life dedicated to service, this award was created to solve a critical problem: the “waiting period” between a veteran’s discharge from a healthcare facility and the time the VA activates Home Health Aide (HHA) services
The Solution
This program provides immediate, temporary home care support at no cost to the client. It ensures that our heroes have professional assistance during those vital first days at home, preventing readmissions and ensuring a safe recovery while VA paperwork is finalized
Eligibility Requirements
To ensure support reaches those with the most critical needs, the award is granted to qualifying veterans who meet the following criteria:
- Clinical Care Requirement
The veteran must demonstrate a clinical need for assistance with Activities of Daily Living (ADLs) through a professional assessment
